Epic's Secure Processes - SMART on FHIR.

A Technical View.

ClearChart.AI is activated by real-time patient events—such as new lab results, visit summaries, or chart updates—launching a secure communication workflow that fits naturally into Epic’s MyChart ecosystem. Patients are notified when new personalized summaries are ready, and access them through familiar, secure pathways. After reviewing the summary, patients can submit feedback directly, enabling continuous improvement through the PAC (Patient Adaptive Communication) engine. 

Automated Notification and Access.

This is how the patient is notified when new information is available—and how they gain secure access to their ClearChart.AI summary of that information.


The process is simple: a secure email or MyChart alert is sent, containing a time-limited link. Clicking the link launches a SMART on FHIR session via OAuth2, routing the patient through Epic’s MyChart login. No new passwords are needed. The session is secure, private, and tied only to their data. The 3 steps are detailed here:


1. New Clinical Data entered into Epic's database
When new clinical entries—like lab results or visit notes—are recorded in Epic, ClearChart.AI automatically generates a summary. It pulls structured data using the same secure APIs that power MyChart.
 

2. Patient is Notified
When a new entry—such as an AVS (After Visit Summary), lab result, or visit note—is added in Epic, patients automatically receive an alert via secure email or MyChart notification. Each message includes a unique access link tied to their session.


This means the link is:

  • Valid only for a limited time—typically minutes to hours
  • Securely connected to the individual user and event via a short-term access token
  • Encrypted and authenticated using protocols like OAuth2, via Epic
  • Single-use—it expires after the session ends and cannot be reused or forwarded
     

In short, it's a temporary, secure key that ensures the patient is authenticated, the access is limited to their data, and no persistent tokens are exposed.
 

3. MyChart-Based Authentication
The unique access link launches a SMART on FHIR session via OAuth2, logging the patient in through their existing MyChart credentials with the same secure protocols Epic already uses.
 

No App Download Required
Patients view their summary in a secure ClearChart AI browser session—no installation, training, or portal change needed.
 

Session-Bound and Ephemeral Access
Access tokens expire quickly and are never reused. No data is stored or cached by ClearChart.AI after viewing.

Functional Components.

  • SMART on FHIR Integration
    ClearChart.AI uses Epic’s SMART on FHIR APIs to access structured patient data—such as lab results, visit summaries, and medications—without modifying the Epic environment.
     
  • PAC DB Summaries
    Clinical data is transformed into Plain Language narratives through the PAC DB (Patient Adaptive Communication Database), drawing on trusted clinical dictionaries, equity-aware phrasing, and adaptive formatting.
     
  • Automatic Activation on Data Release
    When qualifying information becomes available in Epic, ClearChart.AI automatically prepares the patient’s summary. Patients are notified securely via MyChart or email and access it through OAuth2 login.
     
  • Feedback Loop via CPV
    Each summary invites feedback through Clear Patient Voice (CPV), allowing patients to indicate whether the content was clear and helpful. This feedback supports personalization and PX metrics.
     
  • No PHI Storage or AI Training on Patient Data
    ClearChart.AI uses ephemeral, read-only sessions. It does not store, retain, or train on patient health information, ensuring privacy and regulatory alignment.

By embedding patient feedback into the communication loop—and routing it through Epic’s existing systems—ClearChart fosters responsive, human-centered care at scale.
